Find a Provider
Before you try to find a PPO provider, refer to your ID card to find your primary PPO network. Click on that PPO network’s logo to begin your search. Your PPO network also includes access to Mayo Clinic in Rochester, Minnesota, as well as the Rochester Mayo affiliated hospitals.
You also have access to a Travel PPO Network, which you can only access if you have an emergency illness or injury while traveling outside your primary PPO’s service area, or if you are an insured dependent living outside the primary PPO’s service area. Your Travel PPO Network will be what is displayed on your ID card, either Multiplan or PHCS Healthy Directions. Check the front or back of your ID card to find out which Travel PPO Network you may access.
Use of providers in the Travel PPO Network will be considered Out of Network and paid at a reduced benefit level unless you meet the requirements for an emergency/travel exception, or if you are an insured dependent living outside the primary PPO’s service area.
If you have any questions regarding your network or the PPO providers you may access, please contact us at 1-800-322-0160, Extension 27
NOTE: ACCESS TO THE TRAVEL PPO NETWORK IS ONLY AVAILABLE FOR EMERGENCY TREATMENT WHEN TRAVELING OUTSIDE YOUR PRIMARY PPO NETWORK’S AREA OR IF YOU ARE A DEPENDENT RESIDING OUTSIDE YOUR PRIMARY NETWORK’S AREA. Use of providers in the Travel PPO network will be considered Out-of-Network and paid at a reduced benefit level unless you meet the requirements for an Emergency/Travel exception. If you are not sure if you qualify for access to the Travel PPO network, please contact us at 1-800-322-0160, Extension 2758.
Voluntary Networks of Specialty Care Providers
(NOT available to SuperMed PPO Network members)
Pekin Life Insurance Company is pleased to be a partner with three voluntary networks of providers. These networks provide high quality care at reasonable costs. These networks allow you and your family to obtain necessary care, while minimizing out-of-pocket costs. The use of these networks will help reduce the need for premium increases as these increases are a direct result from claim dollars paid by Pekin Life Insurance Company.
