Below are the different networks that we offer. BE SURE TO REFERENCE YOUR PATIENT’S ID CARD TO DETERMINE THE CORRECT NETWORK. Not all networks are available in all areas. Only the network chosen by your patient can be accessed at the in-network rate. If you are unsure of your patient’s network, please call us to confirm the network at 800-371-9622. Failure to use in-network providers will greatly reduce the benefits for their claims.

NOTE: ACCESS TO TRAVEL PPO NETWORKS IS ONLY AVAILABLE FOR EMERGENCY TREATMENT WHEN TRAVELING OUTSIDE OF YOUR PRIMARY NETWORK’S AREA. Use of providers in the travel PPO network will still be considered Out-of-Network and paid at a reduced benefit level unless you meet the requirements for an Emergency/Travel exception.
