Find A Provider
Before you try to find a PPO provider, refer to your ID card to find your primary PPO network. Click on that PPO network’s logo to begin your search. Your PPO network also includes access to Mayo Clinic in Rochester, Minnesota, as well as the Rochester Mayo affiliated hospitals.
You also have access to a Travel PPO Network, which you can only access if you have an emergency illness or injury while traveling outside your primary PPO’s service area, or if you are an insured dependent living outside the primary PPO’s service area. Your Travel PPO Network will be what is displayed on your ID card, either Multiplan or PHCS Healthy Directions. Check the front or back of your ID card to find out which Travel PPO Network you may access.
Use of providers in the Travel PPO Network will be considered Out of Network and paid at a reduced benefit level unless you meet the requirements for an emergency/travel exception, or if you are an insured dependent living outside the primary PPO’s service area.

BRIDGEHEALTH MEDICAL
No, you will not be penalized as this is a voluntary network. You will still need to use an in-network provider based on your current policy. Using the BridgeHealth Network will help reduce the need for premium increases due to the savings involved.
Most likely, no. The BridgeHealth medical network selects those hospitals that rank in the top 25% of all hospitals in the country. These providers are of the highest rated in the U.S and are carefully chosen based on performance, as measured by leading independent rating agencies. BridgeHealth works only with highly efficient doctors and hospitals that have achieved some of the best medical outcomes in the United States.
DBM
You will not be penalized as this is a voluntary network. You will still need to use an in-network provider. Using the DBM providers will reduce the need for premium increases due to the savings involved.
When your physician prescribes an MRI, CT, and/or P.E.T. scan, all you have to do is call DBM at 800-331-5720. They will schedule your procedure in a facility that is convenient for you. DBM will also notify your physician of the date and time of your test and even call to remind you of your appointment and answer any questions you may have. Your physician will then receive your results in a timely manner.
LAB CARD SELECT
No, Lab Card Select is a voluntary network. If you choose not to use Lab Card Select, you still have your PPO network.
Outpatient lab work that can be processed under Lab Card Select includes: blood tests, urine tests, cytology and pathology (e.g., pap smears, biopsies), and cultures.

www.labcardselect.com, or call 1-800-750-1253 to see if there is an approved collection site in your area. If a collection site that meets your needs is available, you can take a completed test order from your physician outlining the tests to be performed to the collection site.
